A Community-Engaged Project to Develop a Sustainable Approach to Improve Child and Family Well-being in Urban and Rural Settings
Overview
Connecting the Dots (CTD) is a family-centered approach that leverages home visiting, coaching, and engagement with community organizations to improve overall family well-being. In CTD, families of young children work with navigators to attain self-identified goals for their family.
How it Works
Navigators will coach families residing in rural and urban areas for up to two years coaching them through early childhood language and brain development strategies along with providing support for parental pathways to family self-sufficiency. Throughout the program, families will be connected to community resources to ensure that they have opportunities for educational advancement and increased economic mobility.
